Our continuum of care provides clients with wrap-around services to access every area of support and ensure we provide personalized care.

Intake
The access point for all of our programs and services begins with our intake process. A staff member reviews your eligibility based on at-risk criteria and provides immediate resources.
Services
Once you’ve received food, essentials, or other basic resources, a case manager will connect you with a variety of programs within our organization or help you secure other local resources.
Evaluation
Lastly, a case manager or staff member will routinely meet with you to review your progress, provide further support, and ensure you’ve stabilized any at-risk criteria.
