Volunteer Waiver for One-Day Service Volunteers

You MUST read and complete this form prior to volunteering!

• I hereby waive any right or cause of action arising as a result of my participation as a volunteer from which any liability may or could accrue against Interfaith Community Services or the staff and directors collectively or individually. Without limiting the generality of the foregoing I agree that this waiver shall include any rights or causes of connection with my activities as a volunteer.
• I attest that I am physically fit, prepared to perform the tasks assigned to me, and will follow instructions including all safety protocols when volunteering at Interfaith Community Services.
• I shall not operate a personal vehicle for volunteer activities unless I have at least the minimum amount of liability insurance required by California law.
• I further agree to use my personal insurance as the primary provider in the event of illness or injury due to my work as a volunteer for Interfaith Community Services.
• I understand I may work with confidential information. I agree to keep such information in the strictest confidence.
• Interfaith Community Services is not responsible for loss or damage to volunteer’s personal property.
• I authorize Interfaith to use photographic images taken during this event for promotional purposes without any further consent or approval.
