Project Care

- Qualifications: Must be age 60 and older, and residents of North Inland and North Coastal San Diego County Residents
Project C.A.R.E. (Community Action Reaching the Elderly) is a safety net program for older persons and persons with disabilities. Case managers maintain close contact and support, empowering individuals to remain as independent as possible in a safe environment.
Supports include:
- In-Home case management to remove transportation and mobility barriers
- Home safety assessments
- Healthcare navigation, including benefit application and clarification
- Emergency food and hygiene provisions;
- Provide resources and referrals for external resources, including transportation and housing resources
- Financial assistance through the Senior Flex Fund for extraordinary expenses

Home Safe
- Number of individuals estimated to be served this year: 250
- Qualifications: Individuals referred via Adult Protective Services throughout San Diego County
The Home Safe program provides intensive case management and housing navigation to persons who are referred by Adult Protective Services (APS) and are at risk of homelessness. Home Safe case managers address housing barriers, behavioral/physical health, and financial and budgetary evaluation to ensure their future housing stability.
Supports include:
- Provide immediate and safe temporary housing, when necessary, including
temporary hotel lodging or independent living homes - Housing navigation to meet client’s unique needs including apartments,
independent living houses, long term care facilities, and voucher application. when appropriate - Intensive case management addressing barriers, including realistic expectations in
today’s market, budget evaluation, income and benefit application aid,
transportation, medical and behavioral health needs, ongoing socialization and heath care needs, community relation issues;. - Flexible financial resources to help with rent, deposits, moving cost,
hoarding/clean up expenses, transportation needs, utility assistance and other necessary expenses related to housing stability; - Work with legal agencies, family/friends, and other County resources for the benefit of the client;
- Referrals to ongoing services that will support long term stability, such as local legal resources, senior centers, meal delivery services, PACE centers, etc.

Minor Home Repair
- Number of individuals estimated to be served this year: 225
- Qualifications: Must be 60 years or older (or have a disability) and residents of North Inland and North Coastal San Diego County, medium to low-income restrictions
Minor Home Repair is a “fix-it” program primarily focused on the safety and security of homes for older adults and persons with disabilities.
Services include, but are not limited to, the following:
- Smoke and carbon monoxide detectors
- Screen repairs
- Minor electric issues
- Minor plumbing issues
- Stair rail repair
- Supply and installation of in-home safety items, such as grab bars, toilet seat lifters, shower chairs, etc.
